At a basic level, the supply of a web form and resulting database based on the content in the attached PDF. I can give any other information as required.
I might be interested in treating this as a module which might then integrate into the rest of my web ‘presence’ which is very scattered at present.
My notes in the PDF read:
- This is based on a current paper form. I DON’T want to reproduce this look, so would welcome different layout which is clean and simple making it as easy as possible to navigate and complete.
- I am open to suggestions for a better way to approach this.
- I have played with www.jotform.com and like the look / functionality, but don’t have time to make it work for me.
- Results from form submission should be able to be emailed to pre-determined addresses that I decide on.
- Results should be available to view online (with password access) and download or print in an easy to view layout.
- Security of personal data must be taken into consideration.
- Possible: client conference programme information could pre-populate an event with speaker details.
- Possibility of further / future integration into other aspects of my business (e.g. file upload site, crew booking database).
- Must work on all browsers old and new.
- I would like to be able to modify content easily when required.
- I would like to have additional information shown as pop-up tool-tips / links.
